For current patients. Sign in to view your upcoming appointments, fill out forms online, pay your balance, and find educational resources.
First time using the portal? Call us at (813) 820-0071 and we'll set you up — it takes about two minutes.
New-patient forms
For your first visit. Print and fill out the patient binder at home, and bring it with you along with your insurance card, a list of your medications, and a photo ID.

Before certain procedures
Informed consent forms
For some treatments — fillings, extractions, dentures, root canals and others — we review a consent form together before we begin, so you understand exactly what the procedure involves. Several forms are also available in Spanish.
You don't need to fill these out ahead of time. We'll go over the right form with you at your visit and answer every question first. If you'd like to read one in advance, call (813) 820-0071 and we'll send it to you.
Checklist
What to bring to your first visit
- Your insurance or Medicare Advantage card — we verify your benefits before treatment.
- A list of your current medications — including vitamins and supplements.
- A photo ID — driver's license or any government ID.
- Your completed patient binder — or come 15 minutes early and do it with us.
